Business Associate Agreement

Because products in this portal may handle Protected Health Information (PHI), a BAA is required before access — on every product, whether it needs PHI or not.

HIPAA Business Associate Agreement (Summary)

This Agreement is entered into between the facility/organization named above (“Covered Entity”) and PDPM Audit Optimization Group (“Business Associate”). The Business Associate will:

Use and disclose PHI only as permitted by this Agreement and required by law.
Implement administrative, physical, and technical safeguards for PHI.
Report any use or disclosure not provided for by this Agreement, including breaches.
Ensure that subcontractors that receive PHI agree to the same restrictions.
Make PHI available for access, amendment, and accounting of disclosures.
Return or destroy PHI upon termination where feasible.
